PARENTAL REMINDER OF REQUIRED PHYSICAL EXAMINATIONS
Pennsylvania Public School Code, Section 14-1402(e), requires physical examinations for students upon original entry into school (kindergarten or new first grader), sixth and eleventh grades.
If you have already turned in a private physical examination to the school nurse please disregard this reminder. Since your own private physician is familiar with your child's medical history, we encourage you to have the physical examination done by your family doctor. Attached to this notice is a Private Physician's Report of Physical Examination of a Pupil of School Age. This should be completed by your child's physician and then returned to your child's school. If your child has had a health examination in the 12 months prior to the start of the current school year, a doctor's report from that exam may be used. If not, this notification is intended to give you time to arrange for an examination.
For Cyber students: A physical examination is still required to be turned in to the school nurse at your child’s home school building. This can be mailed, emailed or faxed by you or the doctor.
