Kindergarten Registration
Registering for the 2025-2026 school year
If your child will be five years of age on or before September 1, 2025, it's time to register for kindergarten!

Registration for the Current School Year (2024-2025)
If your family is new to our district, one of the first things you should do is register your school-age child(ren) for school.
Please call the school your child will attend to make an appointment to register. Contact information for each school is listed to the right (or below if you are on a tablet or mobile device). If you are not sure which school your child will attend, please call or email our Transportation Department at 724-287-8723 or transportation@butler.k12.pa.
Please complete the appropriate registration packet and health history/nursing packet and bring both along with the other items listed in the registration checklist below to your registration appointment.

Registration Checklist
1. Student's proof of age (example: original birth certificate).
2. Current copy of student immunizations (Click here to see PA immunization requirements).
3. Two current proofs of residency (more details below).
4. If applicable, an official transcript
5. If applicable, custody paperwork.
6. Completed enrollment packet.
- Kindergarten: Kindergarten registration forms and other information
- 1st-12th grades: Consolidated packet of registration forms for 1st-12th grades
Registration packets are also available in the main office of each school or the district administration office. Contact information for each school is listed on the right of this page.
7. Complete the health history/nursing packet.
- Students in K-12 must complete the health history information, which is included in the registration packet.
8. The signed Technology Agreement.
Registration for Next School Year (2025-2026)
For parents with a child already enrolled in the district:
The next step is to complete your child's kindergarten registration online. If you already have a child enrolled at Butler Area School District, you can register your new kindergarten student through the online parent portal.

Bring Copies of Important Documents to the School
What information and forms are required to submit my child’s kindergarten registration?
While most of the registration process can be done online, we still need to see hard copies of a few important documents. The documents that should be reviewed and/or completed include:
- Student Health History Form
- Immunization Requirements
- Private Physician Exam
- Private Dental Exam
- Medication in Schools Letter
- Authorization for Medication
- Consent for Release of Records Form
- Student Transcript (if entering grades 1-12)
You will also need copies of the following:
- Your child's original birth certificate
- Your child's immunization records
- Two forms of proof of residency
Drop Off Your Paper Forms
You may drop off your child's paper forms (linked above) at his/her school during the school year. The address for each school can be found on the right (or below if you are using a mobile device). If you are registering over the summer, please drop the forms at the Administrative Building (110 Campus Lane, Butler, PA 16001).
Mail Your Paper Forms
You can also mail your child’s paper forms (linked above) to his/her school. The address for each school can be found on the right (or below if you are using a mobile device).

You can email your child's completed paper forms to the appropriate address for your child's school. Email addresses can be found on the right side of this page (or below if you are using a mobile device).
Proof of Residency
You will need two proofs of residency, one from each list below, to register your child for school in the Butler Area School District. The following documents may be used as proof of residency within the Butler Area School District. Please submit one proof of residency from each list below when you register your child.
