School Forms and Applications
Butler Area School District student forms and applications can be found on this page. This includes all attendance forms (including the vacation form), medical forms, enrollment forms, change request forms, athletic forms, the SHS parking application and more.
The parent-student handbook will give you information about the practices, routines, procedures and policies of our schools. We recommend that you keep this handbook available for reference during the school year. Click to view our student handbooks.
Once your child enters kindergarten, school attendance is required by the Commonwealth of Pennsylvania. Visit our attendance page for multi-purpose excuse forms and other attendance related information, including how to seek attendance support for your child.
Media Opt Out
