Parent/Guardian Required Dental Examination Reminder

PARENTAL REMINDER OF REQUIRED DENTAL EXAMINATIONS

 

Pennsylvania Public School Code, Section 14-1403(a), requires dental examinations for students upon original entry into school (kindergarten or new first grader), third and seventh grades.

If you have already turned in a private dental examination please disregard this reminder. 

Since your own private dentist is familiar with your child's dental history, we encourage you to have the dental examination done by your family dentist. Attached to this notice is a Private Dentist Report of Dental Examination of a Pupil of School Age. This should be completed by your child's dentist and then returned to your  child's school. If your child has had a dental examination in the 12 months prior to the start of the current school year, a dentist's report from that exam may be used. If not, this notification is intended to give you time to arrange for an examination.

For Cyber students: A dental examination is still required to be turned in to the school nurse at your child's home school building. This can be mailed, emailed or faxed by you or the doctor.
